Job Purpose / Job Description

The Staff Nurse / Care Coordinator is accountable for the coordination of nursing care, including direct patient care, patient/family education, and transitions of care. The staff nurse supports professional nursing practice across the continuum of care, promoting the mission, vision, and values of SSMC and providing age-appropriate patient care in accordance with the Nursing Scope of Practice and the Professional Nurses Code of Conduct for Abu Dhabi and the UAE.

Responsibilities

Assessment and Diagnosis
  • Collect and analyze assessment data to determine diagnosis or issues.
  • Assess comprehensive data—including physical, psychosocial, emotional, cognitive, and spiritual needs—to derive diagnosis or issues as defined by the patient's condition.
  • Use evidence‑based assessment techniques and instruments, validating the diagnosis or issue with the patient, family, and health‑care team.
  • Prioritize collected data based on the patient's immediate condition or anticipated needs, identifying potential risks or barriers to health or safety.
  • Document collected data and information in compliance with established guidelines.
Planning and Outcomes Identification
  • Identify expected outcomes and develop a plan addressing health promotion, restoration, and maintenance.
  • Involve patient, family, and health‑care team in formulating a culturally appropriate individualized plan of care.
  • Apply evidence‑based nursing practices in developing the individualized plan to attain outcomes and ensure continuity of care.
  • Modify the plan according to ongoing assessment and reevaluation of the patient's response, status, and outcome indicators.
  • Document measurable goals using standardized language or recognized terminology.
Implementation and Coordination of Care Delivery
  • Implement the plan of care in partnership with the patient, family, and health‑care team, utilizing evidence‑based interventions and treatments specific to the patient's needs.
  • Perform treatments and procedures according to established procedural guidelines and protocols.
  • Administer medications in accordance with organization policies and procedures.
  • Ensure all components of the individualized plan, including modifications, are documented in the patient’s health record.
  • Communicate relevant information across all levels of care and organizations.
  • Demonstrate a working knowledge of resource consumption and practices within a cost‑effective framework.
Health Teaching and Health Promotion
  • Utilize teaching strategies appropriate to the patient’s condition and learning needs.
  • Support patients and families in developing skills that promote, maintain, or restore healthy lifestyles and self‑management.
  • Seek opportunities for feedback and evaluate the effectiveness of teaching strategies, including teach‑back techniques.
  • Document all health education components in the patient health record.
Evaluation
  • Evaluate patient/family understanding and response to the plan of care.
  • Use systematic and ongoing assessment data to revise nursing diagnoses, outcomes, and the plan of care.
  • Involve patient, family, and health‑care team in the evaluation process.
  • Provide comprehensive clinical handover reports to ensure patient safety.
  • Attend patient care rounds and update attending providers on changes in the patient’s condition.
  • Communicate ongoing changes in the patient’s plan of care and unit activity to the care team.
  • Document reassessment of patient status, participation, and any plan revisions.
Evidence‑Based Practice and Research
  • Utilize current evidence‑based nursing knowledge, including research findings, to guide and evaluate practice.
  • Participate in evidence‑based and research activities appropriate to the nurse’s level of education and position.
  • Share evidence‑based and research findings with the health‑care team.
  • Engage in performance improvement projects to evaluate and enhance clinical practice.
Leadership
  • Act as a professional role model promoting a positive, team‑based, organized, and safe work environment.
  • Coordinate and supervise delegated activities, ensuring completion and serving as a resource for other team members.
