Job Description
We are seeking an experienced and patient-focused Case Manager Registered Nurse to coordinate and oversee patient care throughout the treatment journey The Case Manager will work closely with physicians nurses multidisciplinary healthcare teams patients and families to ensure that care plans are effectively implemented resources are appropriately utilized and patients receive safe timely and high-quality care
Key Responsibilities- Assess patients medical clinical and psychosocial needs and develop individualized care plans
- Coordinate patient care across physicians nursing teams allied health professionals and other healthcare providers
- Monitor patients progress and ensure continuity of care throughout hospitalization and discharge
- Review treatment plans and identify any barriers that may affect patient outcomes
- Facilitate discharge planning and coordinate follow-up care rehabilitation home care or other required services
- Communicate regularly with patients and families regarding treatment plans progress and available support services
- Ensure appropriate utilization of healthcare resources while maintaining high standards of patient care
- Maintain accurate and up-to-date patient records case notes and care plans
- Participate in multidisciplinary meetings and contribute to patient care decisions
- Identify high-risk or complex cases and ensure appropriate interventions are implemented
- Support patient education and promote adherence to treatment and discharge plans
- Ensure compliance with hospital policies clinical standards and relevant healthcare regulations
- Bachelor s degree in Nursing from a recognized university
- Valid Registered Nurse license or eligibility for professional licensing
- Previous clinical nursing experience is required
- Previous experience in Case Management, Care Coordination, Utilization Management, or a similar role is required
- Strong understanding of patient care pathways, discharge planning, and multidisciplinary healthcare coordination
- Excellent communication and interpersonal skills
- Strong organizational, assessment, and problem-solving abilities
- Ability to manage multiple patient cases and prioritize effectively
- Strong patient advocacy and customer service skills
- Excellent spoken and written English
- Professional appearance and attitude
