We are currently looking for a Full Time Staff Pharmacist to join our Tribal Health Pharmacy Services C team located at the Chickasaw Nation Refill Center.

Overview

This position is located within the Chickasaw Nation Department of Administration under Tribal and Commercial Health Services under Headquarters. This job is located at Chickasaw Nation Pharmacy Refill Center. The purpose of this position is to provide outpatient pharmacy services to these beneficiaries. Pharmacist must maintain the highest level of attention to detail, such as drug-drug interactions, drug-disease state interactions, drug-patient interactions (i.e. age, allergies, pregnancy, etc.),

and preventing medication errors, all of which have the potential to cause injury or death.

  • Maintains knowledge of and follows all laws, rules, regulations, policies and SOPs for assigned area, e.g. MICS, TICS, hospital standards, health codes, food sanitation, accounting principles, etc.
  • Participates in performance improvement activities to measure, assess and improve the quality of assigned work area
  • Maintains confidentiality, security, and integrity of data and information in accordance with HIPAA regulations
  • Compounds and dispenses prescriptions, check drugs, dosages, dosage forms, labels and provides complete instructions on the correct use and storage of all medications
  • Monitors drug therapy regimens for contraindications, therapeutic duplications, drug-drug interactions, drug-food interactions, drug-laboratory interactions, allergies, and appropriateness of drug and dose
  • Provides drug information to Medical, Dental and Nursing staffs on therapy and drug related matters and patient education as to proper use of drugs. Provides accurate, adequate, and timely drug information to the professional staff. Interprets medical information and applies it to resolve problems or answer questions. Counsels patient and/or family on medications when needed
  • Provides clinical consultation and clarification to practitioners and documents all clinical activities and interventions accurately and completely and detects, assesses, and reports suspected adverse drug reactions. Suggest appropriate, cost-effective therapeutic alternatives to medical staff as needed. Makes dosing recommendations
  • Obtains medication histories from patients, caregivers, and health care providers
  • Maintains due diligence in evaluation of scheduled medications to prevent diversion or patient abuse. Peforms a report on OK Aware to monitor patient scheduled drugs history
  • Maintains accurate and complete patient medication profiles. Complies with federal, state, and local regulations and accreditation standards relating to medication use safety. Provides medication use safety education to patients, caregivers, health care providers, and other appropriate staff. Adheres to safe medication use practices according to established policies and procedures
  • Contacts prescribers regarding unclear prescriptions or problem medication orders. Receives verbal and written medication orders from licensed practitioners and screens medications orders for completeness, accuracy, and clinical appropriateness
  • Works in conjunction with insurance providers, PBM’s, and all payors regarding proper payments of medications based on quantities supplied, days supply limits. Will supply Prior Authorization documents as needed to providers to help patients get needed medications.
