Case Manager, Social Worker – PRN – Lifepoint Health®

The Mercy Hospital in Ada, Oklahoma is a 159‑bed acute‑care facility that provides vital healthcare services to the Ada and surrounding communities.

• Interview and assess patients and/or their family, caregivers, and/or legal representatives.
• Determine, prioritize, and arrange internal and external services or interventions.
• Participate in case reviews to evaluate case‑management progress and consult with the healthcare team to promote compliance with patient treatment plans.
• Assist with discharge planning and identify resources such as transportation, housing, healthcare, and social/spiritual services, providing referrals as part of the discharge plan.
• Mentor new staff, students, and interns.
• Perform Medicaid screenings as required for the NHP program.
• Assist patients with Safety Net applications as needed.

• Bachelor’s degree or equivalent (required).
• Licensed Social Worker – must obtain licensure within six months of hire.
• Current state clinical licensure (required).
• Basic Life Support (BLS) certification required within 30 days of hire.
• Experience with case management, discharge planning, Medicaid screening preferred.
