Overview
We are hiring full-time, part-time, and per diem RN Case Managers to join the Hospice of Western & Central Massachusetts team in Berkshire County! Up to an $8,000 sign-on bonus available. This is an on-the-road position that requires travel and a reliable and insured vehicle. Weekly pay.

Responsibilities- Supervise hospice aides.
- Complete an initial, comprehensive assessment of the patient and family to determine hospice needs.
- Provide a complete physical assessment and history of current and previous illnesses.
- Provide professional nursing care by utilizing all elements of the nursing process.
- Assess and evaluate the patient’s status by writing and initiating a plan of care, regularly re-evaluating the patient and family/caregiver needs, and participating in revising the plan of care as necessary.
- Initiate the plan of care and make necessary revisions as patient status and needs change.
- Use health assessment data to determine nursing diagnosis.
- Develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions, including the patient and family in the planning process.
- Initiate appropriate preventive and rehabilitative nursing procedures and administer medications and treatments as prescribed by the physician in the plan of care.
- Counsel the patient and family in meeting nursing and related needs.
- Provide health care instructions to the patient as appropriate per assessment and plan.
- Assist the patient with activities of daily living and facilitate efforts toward self-sufficiency and optional comfort care.
- Act as Case Manager when assigned by Clinical Supervisor and coordinate patient care for the assigned caseload.
- Graduate of an accredited school of nursing; bachelor’s degree preferred.
- Must possess an active and unencumbered RN license in Massachusetts.
- One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent home care experience, preferred.
- CPR certification.
