Position SummaryWe are hiring full‑time, part‑time, and per‑diem RN Case Managers to join the Hospice of Western & Central Massachusetts team in Berkshire County. This on‑the‑road position requires travel and a reliable, insured vehicle. Up to an $8,000 sign‑on bonus is available. Pay is weekly at $48–$50 per hour.
Experience & Education- Graduate of an accredited nursing school; bachelor’s degree preferred.
- Active, unencumbered RN license in Massachusetts.
- 1–2 years recent acute‑care experience in an institutional setting.
- 1–2 years recent experience in home care (preferred).
- CPR certification.
- Supervise hospice aides.
- Complete an initial, comprehensive, and ongoing assessment of the patient and family to determine hospice needs.
- Provide a complete physical assessment and history of current and previous illnesses.
- Provide professional nursing care using all elements of the nursing process.
- Assess and evaluate patient status, write and initiate a plan of care, and re‑evaluate regularly.
- Initiate the plan of care and make necessary revisions as patient status changes.
- Use health assessment data to determine nursing diagnosis.
- Develop a care plan that establishes goals and incorporates palliative nursing actions, including the patient and family in planning.
- Initiate appropriate preventive and rehabilitative procedures, administer medications and treatments as prescribed.
- Counsel patients and families to meet nursing and related needs.
- Provide health‑care instructions per assessment and plan.
- Assist patients with activities of daily living and encourage self‑sufficiency and comfort care.
- Act as Case Manager when assigned by Clinical Supervisor and coordinate care for the caseload.
