Join DaVita Kidney Care to help patients on dialysis navigate the physical, emotional, and financial challenges of End Stage Renal Disease. As a vital member of the core care team you will advocate for patients’ rights, provide counseling, and coordinate resources to support a smooth transition to life on dialysis.

• Build meaningful and long‑term relationships with patients and their families in an intimate outpatient setting.
• Advocate for patients’ rights and responsibilities and help them understand and manage the demands of End Stage Renal Disease.
• Coordinate care, resources, and support services across the interdisciplinary team to improve patient outcomes.

• Meets all state required regulations to practice in the dialysis setting.
• Master’s degree in Social Work (MSW) required with a specialization in clinical practice.
• Demonstrated knowledge of government and private insurance programs.
• Basic computer skills in MS Word, Excel, PowerPoint and Outlook; proficiency with DaVita-specific applications within 60 days.
