• Provide rounding and manage clinical care of the population at the assigned SNF.
• Round daily and prescribe medications and treatments for residents residing at SNFs.
• Deliver high quality, patient-centered, comprehensive primary care services to patients residing in SNFs.
• Timely documentation of all patient care/encounters.
• Available for phone/secure messaging consultations with facility nursing staff.
• Complete documentation in Theoria’s proprietary electronic health record (EHR) system, Chart Easy™ (and other systems such as Point Click Care , Matrix Care, American Health Tech).
• Be on site on and on time for scheduled shifts.
• Maintain reliable transportation to the facility.

• Rounding time 7 AM‑1 PM local time, during which the provider must be available to nursing staff and address emergency issues.
• Shifts begin around 7 AM local time with flexibility within reason.
• Shifts are daily Monday‑Friday.
• Charting/documentation can be completed off‑site.
• This is an in‑person position.
