Overview
The LTC Charles S. Kettles VA Medical Center is seeking a Full-Time Licensed Practical Nurse for the PACT/Sub-Specialty Clinics at the Adrian VA Clinic. This LPN is aligned under Chief of Staff Services and will be assigned to various Patient Aligned Care Teams (PACT) and/or Sub-Specialty Clinics located at the Adrian, Jackson, Toledo, and Findlay Community Based Outpatient Clinics (CBOCs).
Responsibilities- Total Rewards of a Allied Health Professional The LPN is aligned under Chief of Staff Services and will be assigned to various Patient Aligned Care Teams (PACT) and/or Sub-Specialty Clinics
- This is a PACT/ Sub-Specialty LPN with the official duty station designated as Adrian CBOC
- To support overall health care delivery efforts of the VA Ann Arbor Healthcare System (VAAAHS) - this PACT/ Sub-Specialty LPN may be assigned to one or more clinical areas within the community-based outpatient clinic (CBOC) - and/or offsite ancillary clinic to perform clinical roles - within the scope of their practice (up to 25% of the time)
- Some of the areas/clinics the incumbent would typically be assigned to - but are not limited to - are the Adrian CBOC - Jackson CBOC - Toledo CBOC - and Findlay CBOC
- Duties Include - but are not limited to:
Technically proficient in initiating - performing - and completing assigned duties in providing care to variable patient populations - Knowledge and ability to appropriately carry out assigned patient care based on the patients' conditions
- use judgment in selecting the appropriate order and sequence of procedures and treatments
- and accurately recognize - report and record relevant patient information
- Submits work for general review by a registered nurse or physician for appropriateness and conformity with established policies/procedures
- Ability to observe/respond to the patient's needs for care - including medication and equipment-assisted care (patient/family education)
- In organizing and delivering care - the LPN recognizes and considers factors that impact the patient's care plan
- Prepares and administers prescribed medications (oral - topical - subcutaneous - and/or intramuscular) and performs minor - non-surgical - treatments according to established policies/procedures
- Observes for physical and/or emotional changes in patient s condition from prescribed medication/treatments - promptly and accurately documents notable changes - and reports any deviations from normal to RN or MD/DO
- Knowledge and ability to recognize urgent and emergent patient care situation - seeks assistance of the RN and/or MD/DO - and initiates appropriate emergency interventions as directed
- Establishes constructive relationships with individual patients and their families to elicit feelings and attitudes - and to promotes positive relationships - communication - and socialization skills
- Fosters an environment of respect for individual patient and family rights to privacy and dignity in all aspects of care delivery
- Effectively incorporates knowledge and understanding of established customer service standards in all interactions with patients - family members - and/or other inter/external customers/stakeholders
- Knowledge and skill in performing support duties for basic/standard diagnostic tests and/or practices or procedures - which include preparing the patients - assisting in the diagnostic examination - preparing and handling required equipment - within scope - and monitoring the patient s condition before - during and following the procedure - as deemed appropriate depending on clinic complexity
- Participates in orienting - educating - and training less experienced LPNs/Nursing Assistants (NAs)/or Health Technicians (HTs)
- Participates in mandatory educational opportunities to enhance nursing knowledge and skills to improve and advance own nursing practice

7:30am-4:00pm - Parental Leave:
After 12 months of employment - up to 12 weeks of paid parental leave in connection with the birth - adoption - or foster care placement of a child - Child Care Subsidy:
After 60 days of employment - full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66 - Retirement:

- Basic Requirements:
- Citizenship
- Education:
Graduate of a school of practical or vocational nursing approved by appropriate State agency and/or NLNAC - Licensure:
Full - active - current and unrestricted licensure as a licensed practical or vocational nurse - English Language Proficiency:
Proficient in spoken and written English - Grade Determinations: GS-3 to GS-6 criteria with examples of duties and experience
