As a Registered Nurse Home Health, you’ll contribute to our success in the following ways:
- Ensure that all activities performed align with the vision of Elara Caring’s leadership.
- Maintain patient and staff privacy and confidentiality pursuant to HIPAA.
- Assess assigned caseload to identify patients’ physical, psychosocial, and environmental needs.
- Assume primary responsibility for caseload, including assessment, planning, coordinating, implementing, and evaluating care plans.
- Obtain data on physical, psychological, social, and spiritual factors to tailor patient care.
- Conduct initial evaluations and follow‑up visits according to patient, family, and facility needs.
- Communicate significant findings, problems, and changes in health condition or environment to the Clinical Supervisor, physician, facility, and other personnel involved in care.
- Revise care plans in response to identified patient issues and notify the Clinical Supervisor and other team members.
What is Required?
- Current, unrestricted state RN license
- 1+ year experience in a clinical care setting as a nurse
- Able and willing to travel within the assigned branch/office coverage area
- Must have a dependable vehicle, valid driver’s license, and current auto insurance as required by state laws
- Willing to travel 50%
- Associate’s or Bachelor’s Degree in Nursing preferred
- Previous Home Health or Hospice experience preferred
- Previous experience with Home Care Home Base software preferred
You will report to the Clinical Manager or Clinical Supervisor.
