Position Summary:

Launch your services in Sam's Club. As an Independent Optometrist leasing space inside our stores, you will take a leading role in patient care with respect to eye and vision care as well as general health and wellbeing.

• Establish and control your days of practice, hours of operation, patient schedule, and fee schedule.
• Maintain 100% of examination and professional fees.
• Optionally, opt into advanced equipment/instrumentation and administrative/opt-tech support services, if state allows.

• Doctor of Optometry (OD) from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist (MD or DO) from an accredited school.
• Valid state ophthalmology license.
• General Liability Certificate / Professional Liability Insurance Certificate.

ME/VA license agreements are with Gumberg Asset Management Company.
