Dialysis Registered Nurse, Home Hemo & Peritoneal RN - Sign-On Bonus up to $5,000
The Home Therapy Registered Nurse, as qualified by federal and state regulations, provides patient training and ongoing support for all patients choosing a home dialysis modality. This position is accountable for providing quality care to all patients and is committed to delivering superior customer service in all internal and external interactions.
Qualified candidates with prior peritoneal dialysis or hemodialysis experience may be eligible for up to $5,000 signing bonus - please inquire. Key Duties- Train home hemo or peritoneal dialysis patients in-center and sometimes in the patient’s home.
- Properties of work schedule:
Monday through Friday, 8am to 5pm; modified schedules possible based on patient caseload. - Competitive on‑call pay when placed in on‑call rotation.
- Holidays – Home Training Nurses rarely work on a holiday.

- Home dialysis with 17% of treatments delivered in patients' homes.
- Knowledge of and compliance with federal, state, and local laws and regulations governing home dialysis program and patient care.
- Train patient (and/or care partner) in self‑care peritoneal dialysis or hemodialysis upon meeting regulatory requirements.
- Conduct home visits to assess patient environment, routinely evaluate performance, and improve care.
- Communicate assessment results to physicians, team members, and others as appropriate.
- Document all nursing services in the Electronic Medical Record accurately.
- Participate in infection control monitoring, implementation, and recording.
- Be familiar with emergency equipment and procedures; communicate procedures with patients.
- Ensure clinic and patient supply inventory is appropriate; place orders timely.
- Seek continuous quality improvement through data tracking and root cause analysis.
- Obtain laboratory tests and communicate critical values to the responsible physician.
- Administer medications and complete monthly medication profile reviews.
- Perform on‑call nursing services, nights and weekends, on a rotational basis as needed.
- Flexible with staffing locations and hours to accommodate patient and program needs.
- Coordinate home evaluation with technical services to ensure electrical, plumbing, and drainage requirements meet policy.
- Ensure required water sampling is completed per policy; monitor and report water sample and culture results.
- Current RN license in applicable state, maintained as current and in good standing.
- 12 months of clinical experience in providing nursing care and 3 months of experience in the specific modality (minimum 6 months in Texas). Previous experience with maintenance dialysis preferred.
- CPR certification required prior to patient care assignment.
- Confirm ability to distinguish all primary colors.
- Successfully complete the Home Therapy Nurse Orientation program and maintain annual demonstration of skills and competency applicable for the modality assigned.
- Demonstrated working knowledge of the English language with verbal and written communication skills.
- Proficiency in all U.S. Renal Care clinical applications within 90 days of hire.
- Meet any additional state practice requirements.
- Meet minimum hiring standards, including background clearances and orientation requirements if applicable.
