qualifications

completion of an accredited lpn school of nursing; current licensure with the s.c. State board of nursing

must have a valid driver’s license, and a good driving record as determined by board policy

is supported by three (3) or more documented references from responsible individuals

primary function

to provide quality nursing care, coordinate medical care, and function as a team member to meet the total needs of individuals who reside in icfs/iid. Will safeguard health, safety, respect, and dignity of consumer(s).

functions
  • responsible for implementing physician’s, director of nursing, and dietitian’s instructions in the areas of health maintenance, prevention of illness and accidents, and the administration of medication
  • administers various types of oral, intramuscular, intradermal, or other types of medication on schedules according to approved procedure including documentation of each dosage. The lpn is responsible for accounting for and maintaining safeguards for all medication, in accordance with dhec regulations and tdc policy
  • observes residents for possible side effects from medication, documents observations in record, and reports observations to qidp, residential coordinator, director of nursing, and physician
  • teaches residents about their health needs either formally or informally, as indicated, to meet the individual’s needs
  • the lpn assures that the physicians renew all orders at least every 90 days. The lpn is responsible for coordination of renewal and all necessary appointments prior to the expiration of any order and assures that the physician signs all verbal or phone orders in a timely manner
  • maintains all monthly records for weight, seizure, height, menses, and other nursing assessments and information and performs and documents all nursing treatments
  • observes and inspects the residence, especially the kitchen, for unhealthy or potentially hazardous condition. Ensures infection control standards are maintained in all areas
  • escorts consumers to medical, dental, or therapy appointments either in agency or personal vehicle, as needed, and documents fully in consumer records
  • provides the oncoming duty employees reports of new medical information, and pertinent information on behavioral incidents, etc.
  • assesses residents and reports to residential services manager or qidp/residential coordinator/director of nursing any physical, social, emotional or behavior irregularities
  • responsible for providing input into and implementation and documentation of behavior support plans
  • prioritizes nursing duties with the director of nursing to assure nursing standards are maintained and consults with the director of nursing for nursing guidance as deemed necessary
  • writes summaries and other documentation as required by tri-development center policy, medicaid regulations, and as indicated by health-related issues of the individuals served. Provides needed assessments and participation in individual program plans, new admissions, and discharges, as required by medicaid regulations
  • the lpn must gain thorough knowledge of tdc nursing policy, tdc pharmacy policy, dhec licensing regulations and dhec certification regulations for icf/iid and meet these standards in all phases of nursing
  • assists residents with personal hygiene (bathing, toileting, oral hygiene, etc.), provides formal and/or informal training to non-licensed personnel in proper personal care of residents and provides guidance and supervision of non-licensed personnel in duties relating to health and infection control
  • performs any other related duties when needed

  • attends appropriate in-service training, required meetings, and nurses’ meetings
  • maintains a good working knowledge of agency, scddsn, and governmental standards which apply to his/her position, and has general knowledge of programs operated by tri-development center
