Licensed Practical Nurse in Home Care

Provide community-based nursing care as a Licensed Practical Nurse with Assisted Living Alberta. This role involves case management, client assessment, and care coordination while supporting individuals in their homes.

As a Licensed Practical Nurse on the Integrated Home Care team, you will utilize your nursing skills to assess client needs, plan care, and coordinate services across various care settings.

Your role includes performing nursing interventions and caring for clients while demonstrating a commitment to high-quality, patient-centered care. Expect to be mobile, traveling up to 100 kilometers daily to assist clients in rural areas.

Key Responsibilities:

• Assess client needs and determine appropriate services

• Coordinate transitions of care and develop care plans

• Perform nursing interventions according to client requirements

• Utilize critical thinking for effective care delivery

• Operate technology for documentation and communication

Requirements:

• Completion of an accredited nursing program

• Active registration with the College of LPNs and HCAs of Alberta

• Two years of recent nursing experience in Acute Care

• Basic Cardiac Life Support certification

• Strong verbal and written communication skills
