Do you have excellent communication skills and strive to provide a high standard of care to clients and carers including undertaking assessment, planning and implementation of action plans. Could you visit clients and carers in person or via digital technology and offer emotional support and help signposting and facilitating access to appropriate care and support? Could you help recruit, train and support our Volunteer Befrienders?

St Andrew’s Hospice Community Outreach Service is a 3 year project supported by The National Lottery Community Fund. The project aims to deliver an outreach programme across Lanarkshire supporting individuals and their families impacted by life limiting illnesses. This project will use holistic assessment and a person-centred approach to deliver practical and emotional support whilst also reducing isolation and loneliness, improving overall wellbeing, and assisting in the management of stress and anxiety.
This post is funded by National Lottery Community Fund for 3 years
