The Village of Millers Creek is seeking a skilled Nurse Practitioner to enhance its resident-centred care approach. In this permanent, full-time position, you will lead initiatives aimed at improving clinical quality and support inter-disciplinary efforts. Experience in conducting assessments and mentoring healthcare professionals is vital to promote holistic care for our residents.
Key Responsibilities:
• Complete thorough geriatric assessments for village residents
• Mentor care professionals in delivering high-quality care
• Understand and apply long-term care regulations
• Lead resident-centred palliative care initiatives
• Collaborate with teams on clinical goals and quality improvement
Requirements:
• Registration as a Nurse Practitioner in Ontario
• Experience with comprehensive assessments
• Certification in CPR at Heart Save Level
• Knowledge of regulatory policies in long-term care
• Strong relationship-building skills within a healthcare framework
