Make a difference as a Nurse Practitioner with Age Care, committed to delivering compassionate, person-centered care in long-term settings. Collaborate with care teams to address the diverse needs of residents.
In this impactful role, you will assess and manage the health of residents, working closely with medical staff to develop treatment plans. Your advocacy for residents and family engagement will ensure holistic care tailored to each individual’s needs. Your leadership in clinical areas will support successful transitions and enhance the quality of life for seniors.
Key Responsibilities:
• Assess resident health conditions and create treatment strategies
• Collaborate with MDs and teams for effective care delivery
• Advocate for residents' well-being during conferences
• Provide educational support and leadership to nursing staff
• Participate in effective discharge planning and care transitions
Requirements:
• One year of geriatric or long-term care nursing experience
• Current registration as an RN in the Extended Class with the College of Nurses of Ontario
• Familiarity with long-term care legislation is advantageous
• Leadership capabilities in nursing practices a plus
• Dedication to serving seniors with respect and dignity
