Position: Youth-Centered Nurse Practitioner Role
Elevate youth wellness at Durham Community Health Centre as a Nurse Practitioner, focusing on integrated primary care for diverse youth populations. Engage in both virtual and community-based care practices effectively.

This part-time position spans 17.5 hours weekly, including some evenings and weekends. You’ll provide comprehensive assessments, treatment, and health education to address various health needs like substance use and mental wellness. Key Responsibilities:

• Provide trauma-informed, youth-centered primary care

• Manage pharmacotherapy within NP guidelines

• Conduct comprehensive intake assessments

• Facilitate integration and support for external referrals

Requirements:

• Master’s Degree in Nursing and NP-PHC program completion

• At least 3 years of primary care experience

• Current registration as a Nurse Practitioner

• Preferred experience with marginalized youth

• Access to a vehicle for community service
