Shape the future of youth wellness as a Nurse Practitioner with Durham Community Health Centre, delivering primary care services focused on mental health and wellness support. Embrace a flexible, community-oriented work model.
This permanent part-time role offers 17.5 hours a week, including evenings and weekends. You will join an inter-professional team dedicated to providing comprehensive and trauma-responsive care to youth facing complex health challenges. Your contributions will ensure accessible health services are available throughout Durham Region.
Key Responsibilities:
• Provide comprehensive and youth-centered primary care
• Prescribe medications and manage treatment plans
• Support the integration of care through warm handoffs
• Engage in quality improvement initiatives
• Create a non-judgmental space for youth voices
Requirements:
• Master’s Degree in Nursing with NP status
• 3+ years of experience in complex health settings
• Current registration as RN Practitioner in Ontario
• Knowledge of community services in Durham Region
• Valid Ontario driver’s license for local travel
