Palliative Care Nurse at VHA

VHA Home Health Care is expanding its Palliative Care program and looking for dedicated RNs and RPNs. In this role, you will assess and coordinate care for clients while collaborating with a multidisciplinary team to enhance the quality of life for individuals and their families. You will also manage your schedule and enjoy the flexibility of home health nursing.
Key Responsibilities:
• Assess and plan patient care effectively
• Monitor and evaluate care interventions continuously
• Ensure high-quality patient care delivery
• Maintain accurate client documentation
• Participate in a standby/on-call rotation
Requirements:
• RN or RPN registration in Ontario
• Valid CPR or First Aid certification
• Experience in acute care and community nursing
• Strong communication and documentation skills
• Valid Ontario driver’s license and reliable vehicle
