Key Responsibilities:
• Conduct comprehensive geriatric assessments for residents
• Lead palliative care services tailored to individual needs
• Collaborate with interdisciplinary teams to improve care outcomes
• Educate and mentor care professionals within the village
• Foster strong relationships with residents and families
Requirements:
• Current NP registration with the College of Nurses of Ontario
• Experience in geriatric assessments and clinical settings
• Valid CPR and AIS certifications
• Knowledge of long-term care legislation and policies
• Caring and respectful attitude toward residents and team members
