Registered Practical Nurse

DUTIES AND RESPONSIBILITIES:

* Assessing, planning, implementing and evaluating to provide high quality nursing care to residents;

* Assisting in the development and maintenance of an appropriate plan of care for each resident Ensuring resident care plans are initiated in collaboration with other members of the multidisciplinary team

* Communicating with relatives regarding the care and condition of respective residents as required

* Participating in discharge planning, consultation, and/or transfer of residents

* Providing provide direct resident care and participate in resident teaching as required

* Encouraging active participation by residents in all therapy programs and recreational activities

* Providing first aid to staff when necessary and completing Employee Injury Reports

* Procuring and administer medications including injections and narcotics

* Participating in the orientation, on-the-job training and in-servicing of nursing staff as required

QUALIFICATIONS:

* Current Certificate of Registration in good standing with the College of Nurses of Ontario as a Registered Practical Nurse

* Related experience in a geriatric or Long Term Care environment is preferred;

* Demonstrated commitment to maintain Reflective Practice Standards as outlined by the CNO;

* Demonstrated knowledge of and compliance with safe work practices

* Must possess strong written and oral communication, critical thinking, care coordination, time management and customer focus skills;

* Demonstrated empathy, concern and interest in providing quality care and service for residents

* Physical ability to perform the essential duties of the job

* Must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months;

* Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-services in this area.
