Clinical Pharmacist Specialist – PICU
Summit County, OH. A leading pediatric health system in Ohio is seeking a Clinical Pharmacist Specialist to join its PICU pharmacy team. This role offers a strong clinical focus, interdisciplinary collaboration, and the opportunity to influence medication safety and therapeutic outcomes for a critical care population.
Position SummaryThe Clinical Pharmacist Specialist provides advanced clinical pharmacy services within the Pediatric Intensive Care Unit. This includes evaluating and processing medication orders, monitoring therapy, supporting drug information needs, participating in multidisciplinary rounds, and ensuring safe and effective pharmacotherapy for all assigned patients.
Key Responsibilities- Manage complex clinical pharmacy functions including drug therapy management, PK dosing, protocol development, formulary evaluation, and participation in operational initiatives
- Support nursing and prescribers with medication‑use challenges and cost‑containment strategies
- Evaluate medication orders for appropriate dose, interval, route, duration, organ function, and disease state
- Monitor and document therapeutic response and interventions in the EMR
- Provide patient and family medication counseling
- Precept pharmacy residents/students; deliver presentations and education to pharmacy staff
- Participate in drug‑use evaluations, P&T Committee activities, and accreditation processes
- Serve as a resource for medication safety, adverse drug event management, and system improvement
- Perform centralized pharmacy duties as needed
- Doctor of Pharmacy (Pharm
D) or BS Pharmacy with relevant pediatric experience - Active Ohio pharmacist license
- PGY1 pharmacy residency or equivalent clinical experience
- Proficiency with Epic Willow/Ambulatory and MS Office
- Experience with pediatric/neonatal/adult drug therapy (based on assigned patient population)
- $129,883–$153,212/year, based on years of experience
- $10,000 Sign‑On Bonus, paid on first paycheck
