Akron Children's Hospital is seeking a Case Manager to coordinate hospitalization and discharge planning for a patient caseload. You will work onsite on the Akron Main campus, Monday–Friday, with 8am–4:30pm shifts, and support families through transition care.
The role requires RN licensure in Ohio, BSN preferred, and 3–5 years of case management experience, plus strong communication skills and EMR proficiency. BLS is required.
