Social Services Designee

Green Village Rehabilitation and Wellness is a 72 bed facility with all private rooms located in Green, Ohio.

We are looking for compassionate healthcare employees to join our team. Experienced or newly licensed medical professionals are welcomed to apply. This is a professional position responsible for a wide range of social services to our residents and their families. The services provided may include: problem solving, community referrals, financial education and guidance.

• Function as a support person for the patient/family/caregiver on an ongoing basis.

• Provide and maintain a safe environment for the patient.

• Assist patient/family/caregiver and other team members in providing continuity of care.

• Demonstrates an ability to prioritize work responsibilities effectively.

• Ability to develop and maintain good working relationships within the community by maintaining existing resources and developing new ones.

• Deals with conflict situations in a professional manner

Minimum of 1-2 years in varied multidisciplinary settings in the health care field.
