Registered Nurse (RN) Case Manager – Home Care

Alternate Solutions Health Network seeks a dedicated Registered Nurse to manage home‑based care for patients recovering from acute conditions. The role utilizes nursing expertise, coordination, and compassionate communication to optimize patient outcomes.

Monday – Friday, 8:15am – 5:00pm with Evening & Weekend On‑Call rotation.

Location:

Firestone Park, Akron, Kenmore.

• Provide skilled nursing care in patients’ homes per physician orders.
• Develop individualized care plans; conduct OASIS assessments and adjust plans as needed.
• Administer medications and minimize risk of readmission for high‑risk patients.
• Coordinate multidisciplinary care; manage admissions, transfers, and discharges.
• Educate patients and families on disease processes using teach‑back methods.
• Maintain accurate clinical documentation following agency protocols and Medicare/Federal guidelines.
• Participate in agency in‑service training and mandatory meetings.

• Active RN license in the state of employment.
• Minimum one‑year RN experience in acute care; home care experience preferred.
• Valid driver’s license and auto insurance in the holder’s name.
• Physical capability to meet all job demands.

• Attention to detail; observant and follows directions.
