Male client- needs assistance with daily living needs. Has a G‑tube. Looking for an LPN who can cover the following days:
Tuesdays & Thursdays 4pm-10pm, Fridays 10am-4pm, Every other Fridays 4pm-10pm, Saturdays 10am-2pm OR 10am-4pm (Saturdays can either be every week or every other week.)

Our LPNs provide services in the comfort of the patient's own home and are responsible for delivering quality care, appropriately and compassionately. We are looking to add enthusiastic members to join our independently owned and operated home health agency.

Position Responsibilities

• Assist RN in establishment of plan of care, using sound judgment and critical thinking
• Maintain accurate, detailed, and timely documentation consistent with the plan of care and the nursing processes
• Perform all skills, treatments, and procedures to the scope of practice as defined in the Ohio Nurse Practice Act
• Counsels the client and family in meeting nursing and related needs
• Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds

Qualifications

• Unencumbered license as a Licensed Practical Nurse in Ohio
• Must have graduated from an accredited nursing program and 1 year of nursing experience required (minimally)
• Certified CPR and negative TB skin test or chest x‑ray
• MRDD experience preferred
• Trach care experience
• Reliable transportation and travel time of up to 20–30 minutes required to provide care for our clients (must have active driver's license and proof of auto insurance)
• Drug‑free employees and workplace
