Position: Licensed Practical Nurse (LPN) – Clinical Field Staff Liaison
Schedule: Monday – Friday, 8:15am-5:00pm with 2 holidays per year. Possible evening/weekend coverage for PTO.
Agency: Summa Health at Home and Hospice
Experience: Licensed Practical Nurse with 2 years of experience in an acute care setting and at least 1 year of Home Health experience.
OverviewThe LPN Clinical Field Staff Liaison (CFSL) provides support and acts as a resource to clinicians in the field. The CFSL works in the field with staff and patients and completes visits as needed. ASHN adopts the LPN scope of practice as outlined by the state for its licensed practical nurse personnel. The LPN is responsible for all practices and duties within the scope of practice as outlined by the state.
Qualifications& Attributes
- Licensed Practical Nurse with current license in the state of employment.
- Compact licensure, for states that participate, is required to be obtained within first 30 days of employment for additional support in all ASHN affiliated agencies.
- Two years of experience as an LPN in an acute care setting.
- Minimum one-year home health experience.
- Ability to create positive impressions and communicate with a variety of people. Maintain effective communication with patients, families, physicians and co-workers.
- Ability to remain calm, have patience and be accommodating. Compassionate and caring while working with patients.
- Knowledgeable on nursing best practices. Ability to make appropriate nursing judgments. Ability to identify a situation and handle it with the best possible solution. Detail-oriented and observant.
- Disciplined work ethic. Ability to prioritize and be timely. Works efficiently. Ability to follow directions and work as a team member.
- Serve as a resource for field staff pertaining to clinical issues and/or concerns.
- Facilitate calls to and from Field staff; use resources to resolve issues and report any concerns to manager.
- Work in the field with staff and patients as needed.
- Review Field staff STEPS performance.
- Maintain positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and families.
- Assist with monthly Case Manager, Nurse and Therapy meetings.
- Focus on Outcome Monitors as assigned.
- Complete field visits as needed.
- Participate in managing cost of care by monitoring supply costs and visits per episode.
- Knowledgeable of Clinical Review workflow.
- Review relevant reports. Reviews clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
- Maintains active LPN license in state of employment.
- Participates in on-call, evening/weekend shifts and provides patient care, per agency needs.
- Participates in in-service program development.
- Maintains a professional appearance as a representative of the company.
- Understands and practices policies and procedures for agency operation.
- Arrives at assigned location on scheduled work day. Works according to designated hours.
- Current applicable license as a Licensed Practical Nurse. Knowledgeable with nursing best practices and the scope of practice for a nurse as outlined in Chapter 4723 ORC for its LPN personnel.
- Performs assigned duties with use of computer and phone. Completes and submits all required documentation within specified company requirements.
- Dexterity & vision to complete documentation on computer.
