Sage Group Health Care  Staffing is seeking Registered Nurse Home Health for a 13-week travel/ contract position in Akron, OH. Accepting both local and traveler candidates.

Pay Rate:

Range $2000 to $2280 Weekly Gross pay (Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.50/mile for distance covered between patient home appointments

Shift: 8 Hours;
Days, 8.00a - 5.00p

Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri, 1 weekend day rotation every 5th or 6th weekend. Must meet 32 productivity points per week

• Registered Nurse: 2years
  - Required
• Recent/Current Hospce Nursing in Home Health setting experience: 1 year within the last 3 years
  - Required
• Home Care Home Base(HCHB) charting experience
  - Required
• OASIS experience - preferred
• Must be Covid Vaccinated or provide medical or religious exemption

• ASN or an equivalent
  - Required
• BSN or an equivalent
  - Highly preferred
• RN License for the state applying or compact if applicable
  - Required
• BLS Certification from AHA or American Red Cross
  - Required

• Provide and coordinate care for assigned Hospice patients in Home Health setting
• Manage assigned case load
• Develop and implement nursing care plans for patients as assigned.
• Empower patients and families to reach healthcare goals by educating them on disease management.
• Provide Home Health nursing care and services within the standards and scope of nursing practice asdefined within the state Nurse Practice Act regulations, policies, procedures,and protocols, as well as facility and corporate policies and procedures.
• Ensure physicians'orders are legible if taken verbally and transcribed appropriate perpolicy and procedure.
• Ensure patients'status is assessed on an ongoing basis and pertinent information is gathered and documented.
• Notify physicians of significant changes in status.
• Document and complete charting
• Paid Orientation may last up to 2 weeks

  Northeast part of Red
  - Cuyahoga Falls, Stow, Ravenna and surrounding areas
