Field-based role; travel to client sites or designated locations as required, with occasional office attendance for meetings or training.

The LTSS Service Coordinator manages service coordination for a designated caseload in specialized programs, chooses Person-Centered Planning processes, documents preferences, needs, and goals, and collaborates with Medical Directors and interdisciplinary teams to create comprehensive care plans.

How you will make an impact
  • Perform face-to-face program assessments using predefined tools, applying motivational interviewing techniques to coordinate and manage individual waivers (LTSS/IDD) or mental/physical health needs.
  • Identify members with potential clinical health care needs and coordinate care with the clinical team to provide care‑coordination support.
  • Manage non‑clinical needs of members with chronic illnesses, co‑morbidities, or disabilities, ensuring cost‑effective use of long‑term services and supports.
  • Document short and long‑term service goals in collaboration with the member’s chosen care team, including caregivers, family, and service providers.
  • Identify members who could benefit from alternative service levels or other waiver programs.
  • Serve as mentor or subject‑matter expert for new staff, participate in formal training and process‑improvement initiatives.
  • Submit utilization/authorization requests with supporting documentation aligned with the care plan.
  • Report critical incidents to appropriate state and county agencies (e.g., Adult Protective Services, law enforcement).
  • Assist in appeals, grievances, fair hearings, and state audits.
Minimum Requirements
  • BA/BS degree and at least 2 years of experience working with a social‑work agency or an equivalent combination of education and experience.
Preferred Skills, Capabilities & Experiences
  • Strong preference for case‑management experience with older adults or individuals with disabilities.
  • BA/BS in Health or Nursing preferred.
