Qualified candidates must have at least an associate's degree in human services, urban studies, early childhood, social work or 2+ years' experience in human services, early childhood or similar field. A valid DL as well.
Program Statement:
Summit County Help Me Grow Home Visiting Program is a voluntary program that gives pregnant women and families the necessary resources and skills to raise children who are physically, socially, and emotionally healthy to learn. The goals of the program are to 1. Improve maternal and child health.
2. Prevent child abuse and neglect.
3. Encourage positive parenting; and 4. Promote child development and school readiness.
What You Will Do:
- Build relationships with families to assess their strengths, utilize case management skills determine their needs, and work together to develop goals and connect the family to essential resources.
- Work as an integral member of the Help Me Grow Program by participating in ongoing recruitment as well as completing enrollment applications accurately and timely to determine the eligibility of families for the program.
- Analyze and interpret data to effectively monitor and identify critical areas that will allow the whole family to thrive including family services, health & nutrition, physical and mental health.
Responsibilities:
- Under direction, ensure that the activities of this position and relevant programs are consistent with the mission, vision and values of Catholic Charities, Diocese of Cleveland.
- Engage families and maintain a set caseload as dictated by Ohio Department of Health state rule of parent with children 0-5 years old.
- Determine eligibility of families for program.
- Deliver home visits to eligible families to increase healthy pregnancies, improve their home environment and strengthen parent-child interactions.
- Plan, implement, and facilitate parenting education activities using an evidenced based curriculum.
- Develop realistic and attainable goal plans with family to address child development and social and emotional growth as well as parent/child bonding and positive interactions.
- Review and monitor services, reassess needs and initiate services which as responsive to the needs of clients.
- Assist parents in developing formal and informal community supports.
- Act as an advocate to programs and clients.
- Complete and maintain confidential records and appropriate documentation as required.
- Record data into de-centralized billing system.
- Achieve and maintain credentialing status by completing required training as required by Ohio Department of Health.
