Physical Therapist (Home Health) Job Summary

We are seeking a licensed Physical Therapist to provide in-home physical therapy services in and around Greenville, AL . This role focuses on patient-centered care, functional rehabilitation, wound care, and exercise planning. The PT will evaluate, treat, and educate patients to improve mobility, function, and independence in the home setting.

Physical Therapist (Home Health) Key Responsibilities

• Perform comprehensive physical therapy evaluations per physician orders
• Review patient medical history, diagnoses, precautions, and contraindications
• Develop individualized treatment plans with measurable functional goals
• Deliver skilled physical therapy treatments in patient homes
• Provide wound care and rehabilitation services
• Reassess patient progress and modify treatment plans as needed
• Communicate patient needs and progress with physicians, care teams, patients, and families
• Recommend and assist with adaptive and durable medical equipment
• Maintain accurate clinical and billing documentation
• Ensure compliance with HIPAA, company policies, and state/federal regulations
• Supervise Physical Therapist Assistants and Therapy Aides per state practice act
• Participate in staff development, training, and continuing education
