• Provide 1:1 care for clients following their plan of care while maintaining dignity and independence.
• Administer medications, feedings, oxygen, ostomy care as needed per client’s plan and assess results.
• Accurately document nursing actions and communicate with family, case manager, and physicians, informing them of changes in client’s medical condition and needs.
• Coordinate or order supplies, medications, and feedings as needed to ensure adequate amounts are always on hand.
• Ensure all patient needs are met in accordance with physician instructions.

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice.
• Valid driver’s license, acceptable driving record and proof of car insurance.
• CPR professional certification.
• 1+ years of nursing experience preferred.
• Home health care experience preferred.

The position will initially be employed by LHC Group until at least January 1, 2026, after which employment will transfer to Adoration Home Health / Bright Spring Health Services. LHC Group will handle onboarding and benefits coverage until transfer.
