Title: Hospice Nurse Practitioner
Schedule: Full Time or Part Time Available
Schedule (days/hours): M-F 8:30am-5pm
Territory: Alameda County
Sign On Bonus (Full Time Only): $9,000.00 (Conditions Apply)
Compensation: The posted compensation range of $158,000 - $189,280 (annual salary) is a reasonable estimate that Pathways Home Health & Hospice believes it might pay for this job based on the circumstances hways Home Health & Hospice may pay more or less than the posted range as permitted by law.
Responsibilities- In coordination and collaboration with the assigned Director of Hospice and Hospice Medical Director, evaluate the appropriateness of ongoing hospice care for identified patients.
- Coordinate care with other members of the hospice care team to assure an appropriate plan of care and recertification documentation.
- Serve as a consultant for appropriate transition to hospice care; act as an adjunct to the Hospice Medical Director and as a resource and consultant.
- Provide assessment of select patients to determine appropriateness for home health or hospice.
- Conduct face-to-face assessments in the home setting.
- Coordinate and communicate with Hospice Physicians to ensure ongoing documentation and certification of hospice participation.
- Provide consultative services to Case Managers regarding patients with chronic conditions and/or symptom management issues.
- Assist patients and Case Managers with a smooth transition to Hospice services.
- Document observations, clinical findings, problems, interventions, goals, and plans according to policy.
- Admit patients to service by assessing physical, mental, and social status and eligibility for hospice services according to admission criteria.
- Explain and obtain physician orders for services, equipment, or medications as needed.
- Coordinate with all IDG members (Physician, Social Worker, Home Health Aide, Dietician, Volunteer, Spiritual Care, therapy services) as needed.
- Assist in supervisory visits to nursing staff and communicate findings to the Nursing Manager.
- Document Plan of Care changes as indicated and assess needs for care beyond current certification period, obtaining required orders and documentation.
- Complete discharge procedures and documentation as required.
- Participate in multiple Hospice interdisciplinary team conferences.
- Maintain and update knowledge and skills required to provide quality hospice nursing care.
- Assist in development of hospice protocols, procedures, and policies as requested.
- Participate in QA/PI activities and in-service education related to areas of practice as appropriate.
- Follow established standards for point-of-service technology, documentation, and synchronization.
- Use agency-provided telecommunications devices per established guidelines.
- Submit weekly visit schedules of assigned patients and all documentation per agency requirements.
- Meet established productivity standards and demonstrate established clinical competencies.
- Assist in development of agency protocols, procedures, and policies as requested.
- Assess, develop, organize, and deliver teaching materials for assigned home care patients as appropriate.
- Participate in the development of protocols for patients and evaluate areas for potential inclusion in disease management protocols and outcomes.
- Perform other duties as assigned consistent with skills, training, and Pathways’ mission and goals.
- Nurse Practitioner License Required
- Demonstrated program development and teaching skills preferred.
- Minimum of one year of experience as a professional nurse within the last three years or a baccalaureate degree in nursing from a program accredited by the National League for Nursing.
- Highly motivated, self-directed, flexible, and able to work well in a multidisciplinary team setting.
- Meets competencies per Pathways guidelines.
- Minimum of 5 years of professional nursing experience.
- Willingness and ability to work with electronic input and telecommunication devices.
- Current California driver’s license and automobile insurance with coverage meeting agency requirements.
- Current CPR Certificate.
