Positions in this function support corporate objectives by utilizing a demonstrated competency of Long‑Term Services and Supports (LTSS) to provide case management. You will participate in case conferences with the Supervisor.
Expect to spend about 75% of your time traveling within the assigned territory to visit members in their homes or long‑term care facilities. The target territory includes Conejos, Costilla, and surrounding counties. Flexibility, adaptability, and patience in all types of situations are essential.
Primary Responsibilities- Intake/screening/referral, assessment/reassessment, development of support plans, ongoing case management, monitoring of the Member's health and welfare, documentation of contacts and case management activities in the Department‑prescribed system, resource development, and case closure
- Respect confidentiality and maintain confidences as described in the UHG Employee Handbook.
- Conduct monitoring contacts as prescribed by the Department—at least once within each quarterly period or more frequently as warranted.
- Conduct face‑to‑face contact with the Member at least every six months or more frequently if warranted.
- Complete a new assessment during a face‑to‑face reassessment annually or more frequently if warranted. Complete additional assessments as needed.
- Monitor the delivery of services and supports identified within the Support Plan and the Prior Authorization Request (PAR).
- Attend Department‑provided training for CMA agencies as well as agency‑provided in‑service and staff development training.
- Perform work in accordance with the CMA Contract Agreement's requirements, ensuring accuracy, timeliness, and completeness.
- If you suspect a member of being a victim of mistreatment, abuse, neglect, or exploitation, refer the member to the appropriate protective services or law enforcement.
- Take on special assignments and projects as needed, including system assessment and development.
- Travel is required; the employee must have reliable transportation at their own expense and meet Colorado licensure and insurance requirements.
- Meet one of the following:
- Bachelor's degree in a human behavioral science field such as human services, nursing, social work, psychology, or public health.
- 5+ years of relevant experience in social work.
- Combination of education and experience.
- 1+ years of experience with MS Office (Word, Excel, Outlook).
- Valid driver's license with reliable transportation and ability to travel up to 75% within the assigned territory.
- Reside in or be willing to travel to Conejos, Costilla, and surrounding counties.
- Experience as a caseworker or case manager with an LTSS population may substitute for the required education on a year‑for‑year basis.
- Ability to transition between office and field locations multiple times per day.
- Ability to navigate multiple locations/terrains to visit members and providers.
- Ability to transport equipment to and from field locations (e.g., laptop, stethoscope).
- Ability to remain stationary for long periods to complete computer or tablet duties.
Pay is based on several factors, ranging from $23.41 to $41.83 per hour for full‑time employment. Benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution. All benefits are subject to eligibility requirements.
