Case Management Support I

Hiring Range: $18.31 to $23.81

Pay Range: $18.31 to $26.55

Case Management Support II

Hiring Range: $20.19 to $26.25

Pay Range: $20.19 to $29.27

Hiring Incentive:

This role includes a 30% rural differential to account for cost of living.

Summary of Job Responsibilities:

The Southcentral Foundation (SCF) Case Management Support role involves coordinating and managing administrative duties for a Primary Care Provider (PCP) team and the wider Integrated Care Team (ICT). The CMS acts as a primary contact between the customer-owner and ICT, ensuring effective schedule coordination, communication, information transfer, and system coordination in collaboration with the core medical team.

This position has two levels, designed to provide progressively more responsible and independent work experiences. Advancement between levels depends on demonstrated ability to handle more complex responsibilities.

Qualifications:

SCF programs primarily serve Alaska Native people affiliated with Cook Inlet Region, Inc. (CIRI), and Alaska Native and American Indian people within SCF’s geographical service area. Employees should have a thorough understanding of the cultures and needs of this population to support SCF’s vision of a Native Community that enjoys physical, mental, emotional, and spiritual wellness, and its mission to achieve wellness through health and related services.

  • High School diploma or GED.
  • Bachelor’s degree, or four years of administrative support experience, or an equivalent combination of education and experience, or demonstrated proficiency as an Administrative Support III at SCF.

Additional Qualifications for Case Management Support II:

  • Two years of Case Management Support experience, or demonstrated proficiency as a Case Management Support I at SCF.

Native Preference:

Under P.L. 93-638, as amended, SCF pursues a policy of Native preference in hiring, contracting, and training. Certification from SCF Human Resources is required before applicants receive preference.

Employee Health Requirements:

Compliance with SCF’s Employee Health Procedure is mandatory. All employees must agree to comply with job-related health screenings and immunizations before starting employment. For jobs designated as Health Care Personnel (HCP), documentation of immunizations such as MMR, Varicella, Hepatitis B, Influenza, T-dap, and COVID-19 vaccination is required prior to employment.
