The Outpatient Behavior Health Case Manager-SST3 will provide case management services to individuals with behavioral health diagnosis, abide by agency and state standards around service delivery and documentation of services, provide life skill teaching and linkage to valuable resources, and connect individuals to natural supports in line with individual goals.
Responsibilities:
- Provide case management services to individuals with behavioral health diagnosis
- Abide by agency and state standards around service delivery and documentation of services
- Provide life skill teaching and linkage to valuable resources
- Connect individuals to natural supports in line with individual goals
- Complete weekly case management spreadsheets
- Ensure that other requirements for care are up to date
- Participate in agency, state, and other trainings
- Participate in multidisciplinary meetings, staff meetings, and agency-related meetings
Qualifications:
- Bachelor's in a related field or 3 years' experience
Work Environment:
The work requires the knowledge and skills necessary to provide direct individual services appropriate to adults with mental health diagnoses and/or addictive diseases diagnosis.
