SST III Day Services Care Plan Coordinator

Positions Supervised: None

• High school diploma or GED with one year experience in social or direct care services to special needs groups, OR associate degree in a related field, OR one year experience at a lower level position equivalent.
• Basic reading, writing, and math skills; able to lift 50 lbs; complete Crisis Intervention training; use a computer; maintain a valid driver’s license and auto insurance.
• Knowledge of agency confidentiality policies, individualized plans, documentation, state/federal/local laws, and community resources.
• Ability to observe/monitor behavior, assist with daily living activities, develop individualized plans, conduct assessments, plan and lead recreational activities, provide training/help with personal hygiene, communicate orally and in writing, resolve complaints, and complete evaluations.
• Conflict resolution skills; prepare reports; identify facility repairs; operate state vehicles safely; perform housekeeping duties.

• Fire extinguisher, first aid kit, vans (with or without lift), adaptive equipment (wheelchair, Hoyer lift), computer.

• Employee routinely performs tasks involving exposure to blood or other potentially infectious material (Category I).

• Safety
  • Keep assigned areas, walkways, and fire exits neat and clear of debris.
  • Complete daily cleaning protocol.
  • Store cleaning products in locked cabinet.
  • Report potential infections to supervisor.
  • Provide CPR, First Aid, CPI, or AAA as needed.
  • Assist people supported with transportation to and from the bus.
• Supervision of Individuals Supported through I/DD Services
  • Maintain a staff‑to‑people ratio no greater than 1:10.
  • Maintain line of sight where applicable.
  • Apply enhanced supervision where applicable.
  • Report behavioral concerns to supervisor.
  • Develop training activities relevant to the ISP.
• Documentation
  • Complete tracking daily by shift end.
  • Include goal progress and supporting activities.
  • Write person‑centered notes detailing choices, responses, staff involvement, lessons learned, and future plans.
  • Correct errors line‑through, initials, date.
  • Complete supplemental logs as required (seizure, elimination, behavior, repositioning).
  • Follow frequency and intent outlined in ISP.
• Engagement with People Supported through Routine Day Services
  • Build instructor‑to‑individual relationships.
