Position Summary:
The Case Manager Supervisor oversees and supports a team of case managers to ensure the delivery of high-quality, patient-centered care. This role is responsible for supervising daily operations, coordinating care plans, ensuring compliance with regulatory and organizational standards, and promoting effective communication among interdisciplinary teams.

Job Duties:

• Supervise, mentor, and support a team of case managers
• Oversee daily case management operations and workflow
• Ensure compliance with regulatory, accreditation, and facility standards
• Review and monitor care plans for quality, accuracy, and timeliness

Note: 4 years of Supervisor Experience

Requirements:

• Experience: 4 years
• Certifications: BLS AHA, RN License GA
