Responsibilities Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care Collaborate with leadership to review care coordination initiatives

Develop intervention plans, and share ideas about network and risk management issues

Work alongside pharmacists on medication management

Implement an individualized patient care plans

Timely response in chart completions and pharmacy recommendations

Provide oversight and mentorship to our clinicians in the greater Albany, New York, area Experience as a medical director in a skilled nursing facility preferred. However, clinicians who are board certified and experienced in the following disciplines are encouraged to apply: family (FM), primary care, internal (IM), emergency, (EM), adult, post-acute, home health, hospice, palliative, urgent care, and hospital medicine (HM).Position
