General ResponsibilitiesAs a member of the multidisciplinary team, you will provide psycho‑education, counseling and targeted interventions, facilitate integrated services, conduct comprehensive assessments and develop individualized, integrated treatment plans to empower patients.
Specific Responsibilities- Conduct screenings and biopsychosocial assessments for all patients seeking addictions treatment.
- Develop patient‑centered, culturally and linguistically sensitive treatment plans in collaboration with the multidisciplinary team.
- Administer PHQ‑9, GAD‑7, SDOH, SBIRT, and other assessments as indicated.
- Collaborate with primary care providers, care coordination, and other service agencies.
- Provide case management services and coordinate care among service providers.
- Maintain knowledge of SUD, harm reduction, motivational interviewing, and evidence‑based practices.
- Conduct psychoeducation, individual and group sessions.
- Assess readiness for change, identify barriers, and facilitate referrals to community resources.
- Determine ongoing care and refer complex cases to higher levels of care as needed.
- Maintain accurate, timely documentation in the electronic medical record per agency policy.
- Ensure patients receive swift access to care and meet performance outcomes.
- Participate in departmental case conferences and quality‑assurance activities.
- Network with community providers to support continuum of care.
- Engage in ongoing education, training, supervision and staff meetings.
- Use DSM‑V for diagnostic impressions and maintain caseload.
- Complete continuing education and licensing requirements.
- Adhere to professional ethics, HIPAA, and state/federal regulations.
- Contribute to a safe and respectful treatment environment.
Minimum Qualifications
Master’s Degree in Social Work or Mental Health Counseling from an accredited institution;
New York State licensure as a LMSW or LMHC; at least one year of experience in behavioral health/SUD counseling; strong assessment and diagnostic skills; knowledge of recovery concepts and community resources; excellent customer service and cultural competency; confidentiality compliance; computer and EHR proficiency; NYS Central Registry clearance.
Experience working in a community health care setting.

Salary range: $59,000 - $68,000 annually.
