Trinity Health’s Community Hospice in Albany/Rensselaer Counties is seeking a full-time Social Worker to join the hospice care team. The role covers home and nursing home settings, focusing on emotional and psychological support within an interdisciplinary team. Schedule is Monday–Friday, 8:00 a.m. to 4:30 p.m.
The professional will diagnose and treat end-of-life emotional needs, provide therapy, identify barriers to function, and help secure appropriate resources for patients and families.
