Behavioral Health LPN

• Conduct screenings, including HIV risk screenings, intake screenings, and obtain family health history.
• Routine screening for substance abuse, developmental health, mental health, and coordinate physical body checks for abuse.
• Routine management and training regarding health education.
• Establishing treatment goals, utilizing historical & current available information in collaboration with multidisciplinary team.
• Reinforce instruction of medication for youth and both birth and foster families on medication administration.
• Assist Psychiatric Mental Health Nurse Practitioner:
• Complete Prior Authorizations
• Call pharmacies for medication verification
• Call/ fax labs to schedule lab draws and get results
• Call providers for follow up and records release
• Educate families and youth of safe medication storage and possible side effects to watch for.
• Medication reconciliation and monitoring of medication.
• Support youth and families on how to maintain medical supplies and equipment.
• Follow up after medical/dental appointments, community provider visits, urgent/emergency care, or hospitalization.
• Medical Case Management and documentation for medical, dental and behavioral health appointments and updates.
• Maintains compliance with state and agency law and regulations.
• Adheres to agency policies and procedures; follows and supports agency mission, vision and values including participating in internal surveys and data collection for continuous improvement initiatives.
• Practicing proper infection control.
• Performs other duties as assigned

• Current New York State Licensed Practical Nurse license and CPR/First Aid certification.
• Minimum 1 - 3 years of experience. Experience with at-risk youth preferred.
• Flexible schedule and ability to provide weekend, holiday, and On-Call coverage.
• Must have current, valid driver's license per Agency guidelines. Travel required.
• Ability to work with a culturally and economically diverse population.

• $59,500 per year
