Responsibilities- Develop a working knowledge of FACT as an evidence-based model and practice according to the guidelines set forth by OMH. Participate in daily morning meetings to review client progress, create visit schedules, and build team relationships.
- Conduct person-centered comprehensive health assessments, including personal and family medical and psychiatric histories, current medical needs, substance use history, and daily living skills to link and collaborate with providers to improve health outcomes.
- Develop and implement strength-based treatment plans responsive to client needs and facilitate goal achievement on an as-needed basis.
- Oversee all aspects of medication management in compliance with agency procedures and protocols, including administration of injectable medications, managing Clozaril protocols, ordering medication, overseeing medication distribution, using a medication step system for client self-management, and managing the medication room.
- Provide support, treatment, case management, and community advocacy and linkage services per the treatment plan. Complete required documentation of interventions and client responses/outcomes.
- Readily respond to all crisis situations using sound clinical judgement to ensure safety. Provide on-call crisis intervention services on a rotating schedule among team members.
- Travel to all facility levels and visit client apartments on second and third floors. Provide transportation to clients when necessary. Hold a valid NYS driver's license and be approved by the agency's vehicle insurance carrier to use agency or personal vehicles for business purposes.
- Licensed Registered Professional Nurse by New York State.
- Bachelor's degree in nursing (preferred).
- Three years' experience in a psychiatric setting, with at least one year in an outpatient setting (preferred).
- Strong commitment to the rights and autonomy of individuals with mental illness.
- Ability to work with all people and establish supportive, trusting relationships.
- Upheld individuals' rights and personal preferences; treat everyone with respect and dignity.
- Valid, insurable driver's license and willingness to use own vehicle for business.
- $3,269.24 biweekly ($85,000.24 annual equivalent).
- Rotating on-call with $210/week stipend.
