RN HOSPICE-Care Manager for Nursing Homes in the Capital Region. Come experience the rewards of working in end‑of‑life care! NO WEEKENDS, NO ONCALL.
We are looking for a full‑time RN to work as part of our Hospice Care Team caring for patients and families in local Nursing Homes. The work schedule is Monday – Friday, 8 a.m. – 4:30 p.m.
Position Highlights- Case managers are assigned specific nursing homes to minimize travel
- NO WEEKENDS, NO ONCALL
- National pre‑taxed mileage reimbursement
- Meetings and charting conducted via supplied laptop
- Comprehensive orientation provided

- Case manage patients in the nursing home setting and respond to symptom management needs of hospice patients, coordinating with the team to provide added support to residents and families.
- Work days are Monday to Friday, 8 a.m. to 4:30 p.m. No weekends.
- Oversee and collaborate with the nursing home staff in the care of the hospice patient.
- Current license to practice as a Registered Nurse in New York.
- Associate's Degree in Nursing; BSN preferred.
- Minimum 1 year experience as an RN; long‑term care experience highly preferred.
- Previous hospice experience preferred but not required.
- Valid driver's license and reliable vehicle.
- Effective verbal and written communication skills.
- High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers.
$35.00–$47.75 per hour. Pay is based on experience, skills, and education. Exempt positions are paid within the base salary equivalent of the stated hourly rates. The pay range may vary by location.
