• Utilize the nursing process to diagnose and treat patient responses to actual or potential problems.

• Complete admission assessments per guidelines and interview residents and significant others to gather health history.
• Utilize resources to assess residents with new, unfamiliar, or uncommon conditions.
• Analyze assessment data to determine acute psychological or psychosocial risk and document findings.
• Educate residents about care or services to be provided.
• Develop and contribute to a care plan based on identified problems, expected resident responses, and the medical plan of care.
• Communicate the care plan using action plans, goals, and time frames.

• Current RN license in New York.
• Associate’s degree in nursing or Bachelor of Science in Nursing from an accredited school.

• At least 1 year of experience in a skilled nursing setting.
• Bachelor's degree in nursing.
• Passion for patient care and a respectful, caring, professional demeanor.
• Excellent time‑management and organizational skills.
• Strong verbal and written communication and critical‑thinking skills to problem‑solve.

Pay Range: $35.00 – $47.75 per hour (based on experience, skills, and education). Exempt positions are compensated within the base salary equivalent of the stated rates.
