- Non‑mandated nursing practices.
- Convenient location close to Latham, Colonie, and Delmar.
- Utilize the nursing process to diagnose and treat patient responses to actual or potential problems.
- Complete admission assessments per guidelines and interview residents and significant others to gather health history.
- Utilize resources to assess residents with new, unfamiliar, or uncommon conditions.
- Analyze assessment data to determine acute psychological or psychosocial risk and document findings.
- Educate residents about care or services to be provided.
- Develop and contribute to a care plan based on identified problems, expected resident responses, and the medical plan of care.
- Communicate the care plan using action plans, goals, and time frames.
- Current RN license in New York.
- Associate’s degree in nursing or Bachelor of Science in Nursing from an accredited school.
- At least 1 year of experience in a skilled nursing setting.
- Bachelor's degree in nursing.
- Passion for patient care and a respectful, caring, professional demeanor.
- Excellent time‑management and organizational skills.
- Strong verbal and written communication and critical‑thinking skills to problem‑solve.
Pay Range: $35.00 – $47.75 per hour (based on experience, skills, and education). Exempt positions are compensated within the base salary equivalent of the stated rates.
