Physical Medicine and Rehabilitation - Nurse Practitioner – Troy, NY

St. Peters Health Partners, is seeking to recruit a full time Nurse Practitioner to join our dedicated car team at Eddy Heritage House Nursing and Rehabilitation Center in Troy, NY. We proudly serve seven nursing home facilities across the Capital District, with this opportunity focused on delivering personalized, high-quality care to the 80 Long Term Care residents at the Eddy Heritage House in Troy, NY.

  Enjoy work-life balance with a reliable Monday-Friday schedule of five 8-hour shifts.
• Manageable Caseload:
  See 12-14 residents per shift, including both routine and sick visits.
• Minimal On-Call:
  One week every 6-8 weeks, phone calls only.
• Strong Interdisciplinary Support:
  Collaborate with a robust on-site team including Pulmonary, Physiatry, Psych, Podiatry, WOCN, and more.

Applicants should have the following:

• Were seeking someone who is passionate about geriatric care and thrives in a team-oriented environment.
• Board Certification as a Nurse Practitioner
• Active NYS License and DEA
• Experience working with the geriatric population
• Geriatrics certification is a plus, but not required
• Strong communication skills and a commitment to holistic, compassionate care
