St. Margaret's Center, an affiliate of the Center for Disability Services, is a 94-bed pediatric and young adult skilled nursing facility in Albany, NY. Since 1883, we have provided compassionate, specialized long-term care for medically fragile infants, children, and young adults in a nurturing, home-like environment.

As an LPN garet's Center, you'll provide hands-on nursing care while working closely with RNs, providers, and an interdisciplinary team to support each resident's individual needs.

• Provide direct nursing care in accordance with individualized care plans and physician orders
• Administer medications and perform clinical treatments and procedures within your scope of practice
• Monitor residents' health status and promptly communicate changes or concerns to the RN
• Respond to medical needs and emergencies and provide appropriate interventions under RN direction
  
  Complete accurate and timely nursing documentation and maintain up-to-date medical records
• Assist RNs with assessments, care planning, and annual evaluations as needed
• Ensure nursing equipment is properly maintained and functioning
• Collaborate with nurses, providers, direct care staff, therapists, and other interdisciplinary team members
• Participate in nursing meetings, program initiatives, and ongoing quality improvement efforts

• Current and valid New York State LPN license
• Minimum of one year of experience in an acute care or long-term care setting
• Must be at least 18 years of age
• Ability to safely lift a minimum of 50 lbs.
