Trinity Health is seeking a Full-Time Licensed Practical Nurse (LPN) to join our Hospice Nursing Home Team in the Capital Region. Schedule is Monday through Friday, 8:00 a.m. to 4:00 p.m., with no on-call or mandated overtime.
Provide compassionate hospice care to residents in nursing homes and collaborate with an interdisciplinary team to support patients and families. Ideal candidates hold a NY State LPN license with 1 year of LPN experience; homecare/hospice experience preferred.
