Trinity Health is seeking a full-time Hospice RN to join our Homecare Team serving the Albany/Schenectady County region. You will collaborate with a multidisciplinary group to deliver end-of-life care in patients' homes, Monday through Friday, 8:00 a.m. to 4:30 p.m., with no weekends or on-call.
Responsibilities include coordinating care plans, supporting families, and ensuring high-quality, compassionate care. RN license in NY is required; ADN required with BSN preferred.
